Below are explainations of the different forms you can print by clicking on the above PDF printable form button and choose the form you need to print off.  You will need to have ADOBE READER to load and print off the forms.

CORRECTION OF CERTIFICATE FORM   Form to change name, update picture or correct errors on Registration Certificate.

DNA KIT REQUEST FORM  Form to request DNA KIT for record the DNA of your horse.

ENROLLMENT FORM  Form to enroll your Quarter Horse, Paint or Thoroughbred stallion or mare to qualify their offspring for registration into The American Appendix Horse Association.

HARDSHIP REGISTRATION  Form to register your horse into The American Appendix Horse Association under the hardship clause.

MEMBERSHIP  Form to become a member or renew your membership.  This form can be use to become a member and register a horse at the same time.  If you are using it to just become a member or renew your membership to not fill out the horse iformation.

PERFORMANCE CARD Form to apply for the current year Performance Card to compete in events to earn points and awards at the end of the year.

REGISTRATION Form to become a member and register your horse at the same time.  It can also be used to register an additional horse after you have become a member, just indicate your current membership number and only pay the registration fee.

STALLION BREEDING REPORT This is a two part form, print both page one and page two.

  Form to report your stallion breeding services, this will generate a "Breeding Certificate" for you to mail to the mare owners.  With this they can register the foal.

TATTOO SEARCH Form to do an Appendix tattoo search or a Thoroughbred tattoo search.

TRANSFER OF OWNERSHIP  Form to transfer ownership of your horse.
